Q. What do
I need to bring to my appointment?
A. Insurance card, driver's license or photo ID,
list of current medications, any recent x-rays you
may have, pertinent information about your medical
history, any recent bloodwork results, and a
physician referral form if required by your
insurance.
Q. Do I have to bring a list of my medications?
A. Absolutely.
Q. What insurance do you accept?
A. We accept Medicare, Blue Cross Blue Shield, and
most commercial insurance.
Q. What if I do not have insurance?
A. Payment is due at the time of services.
Q. Are your doctors board certified?
A. Yes, both of our doctors are certified by the
American Board of Urology and the Fellows of the
American College of Surgeons.
Q. What forms of payment do you accept?
A. We accept cash, check, Master Card, Visa, and
American Express (No Discover).
Q. What hospitals do you have privileges to?
A. Providence Hospital, Mobile Infirmary, and Spring
Hill Memorial Hospital.
Q. Do you accept e-mails?
A. For insurance purposes and in order for us to
respond in a timely fashion, we do not accept
e-mails. We recommend that you call our office
at (251)-378-3000.
Q. Do I have to make an appointment for a lab?
A. No, but we do not have labs during the hours of
12:00P.M.-1:00P.M. Our labs are also closed on
Tuesdays before 1:00P.M.
